Job Description:
Performing skilled nursing services including:
wound care, lab work, patient education, etc.
Monitoring and recording patient's vital signs as needed; reporting abnormal readings to the supervisor Observing patient's functional level; reporting any changes in the member's physical condition and health care needs to the supervisor Assisting patients, as needed, with medications; observing for possible reactions; reporting any reactions to the supervisor Completing an in-home supervisory visit with assigned members at least once per year to observe and monitor the in-home performance of the personal support aides Preparing progress notes and supervisory visit entries, documenting patient's current status Collaborating with the supervising RN to update patient care plans Job
Qualifications:
Must be willing to work in patient homes Graduate of an approved Professional Nursing Program.
Current LPN license, with no sanctions, restrictions, or provisions, in the state of Georgia.
One year of related experience in a clinical environment
Benefits:
